• Key Responsibilities:
• Making outreach calls to patients.
• Assisting patients in scheduling follow-up appointments with their primary care providers.
• Engaging with patients in conversations about managing chronic conditions such as diabetes, hypertension, and other health issues.
• Collecting and inputting essential health data to support effective care coordination and improve patient outcomes.
