What You’ll Actually Do
• Assess: Evaluate the physical, emotional, socio-economic, and environmental needs of patients and their families.
• Provide: Deliver direct skilled nursing care in accordance with physician orders and ensure timely documentation of all care provided.
• Monitor: Observe, evaluate, and report the patient’s physical and mental condition, including changes, to the responsible physician.
• Educate: Offer counseling and education to patients, their families, and caregivers to support ongoing health management.
• Collaborate: Develop, implement, and evaluate individualized plans of care in partnership with patients, representatives, caregivers, and physicians under clinical guidance.

The Must-Haves
• Background: Registered Nurse (RN) licensure and an early career focus on home healthcare case management, with an interim period for achieving independence.
• Experience: Minimum one year of experience as a Registered Nurse, with specific training and support provided for new graduates to reach full independence as a case manager within six months.
• Skills: Strong research capabilities, clear and concise interpersonal communication skills, and knowledge of systems integration.
• Bonus: Bachelor’s Degree in Nursing; prior experience in a qualified home health agency is desired.
