• Mission: To manage a patient panel and provide comprehensive, culturally competent primary care, supported by a new grad program designed for continuous learning and team collaboration.

What You’ll Actually Do
• Patient Care Management: Manage a diverse patient panel by conducting a mix of acute, chronic, and well visits with the support and mentorship of an onsite clinical Preceptor.
• Service Delivery: Treat patients across various settings, including in-office, in testing centers, and through occasional telehealth visits.
• Professional Development: Engage in continuous learning during weekly Didactics with dedicated faculty, Clinical Rounds, and through other modalities.
• Team Collaboration: Work closely with in-office teammates via daily huddles and collaborate with virtual clinical teams to ensure cohesive patient care.
• Clinical Procedures & Licensure: Utilize specific clinical training to perform in-office procedures and be willing to obtain additional state licensure and credentialing for One Medical virtual primary care in additional states.

The Must-Haves
• Background: Early Career Nurse Practitioner or Physician Assistant with national certification, having completed an accredited program with a graduation date between May 2026 to December 2026. This role focuses on providing primary care.
• Experience: Must hold an active license in the state of New York or have submitted an application for licensure prior to December 1, 2026.

• Demonstrate a passion for culturally competent primary care and successfully communicate with and provide care to individuals of all backgrounds.
• Possess the ability to effectively use technology to deliver high quality care and to communicate effectively with patients and team members.
• Exhibit clinical proficiency in evidence-based primary care and a desire to be an integral part of a team dedicated to changing healthcare delivery.
• Show an openness to feedback and reflection to gain productive insight into strengths and weaknesses, confidently navigate uncertain situations, and adapt personal and interpersonal behavior to meet the needs of patients.
