What You’ll Actually Do
• Deliver: Provide high-quality, comprehensive nursing care to individuals and families across all life stages, operating at the top of licensure.
• Collaborate: Support planned care models by collaborating with Primary Care Providers (PCPs) to identify needs for preventive care, evidence-based chronic disease care, and care coordination.
• Educate: Provide general and specific health care education to patients, working with them to identify and monitor self-management goals.
• Coordinate: Assume responsibility for coordinating patient care, supporting transitions between different levels of care, and scheduling tests and referrals for specialty care.
• Administer: Safely administer medications, including vaccines and immunizations, and perform nursing activities such as wound care and catheterization, adhering to protocols and orders.

The Must-Haves
• Background: Minimum of an Associate of Science in Nursing (ASN) or a Bachelor of Science in Nursing (BSN), with a focus on primary care nursing for an early career role.
• Experience: Foundational nursing practice gained through an accredited nursing program, demonstrating clinical competence in patient care, medication administration, and health education within various clinical rotations.
• Skills: Proficient in patient assessment and care delivery, medication administration (including vaccines), health education and counseling, care coordination, and accurate electronic health record documentation.
