Job Description
This is a remote internship opportunity within South Shore Health’s Population Health team, designed for students passionate about healthcare and making a real-world impact.
Key Responsibilities (What you’ll do):
• Patient Outreach: Initiate outreach calls to patients.
• Appointment Scheduling: Assist patients in scheduling follow-up appointments with their primary care providers.
• Patient Education & Discussion: Engage with patients about various chronic conditions, including but not limited to diabetes and hypertension.
• Data Collection & Input: Collect and accurately input crucial health data to support care coordination efforts and improve health outcomes.
