• Team: Member of the patient care delivery team, reporting to a registered nurse.
• Mission: This role ensures comprehensive patient care and education is delivered effectively under the guidance of a registered nurse.

What You’ll Actually Do
• Collaborate: Ensure consistent, quality care and foster productive team relations by working with care team members and adhering to DMC customer service standards.
• Coordinate: Oversee patient care activities, including patient and family education, for assigned patients, utilizing clinical pathways, protocols, and incorporating discharge planning.
• Provide Direct Care: Deliver direct patient care by determining patient and family needs, identifying goals, executing physician orders, and documenting patient responses to care and treatment.
• Partner: Work with physicians, care management specialists, and other healthcare providers to develop, implement, and evaluate the patient’s plan of care.
• Educate & Plan Discharge: Identify learning needs of patients and families, initiate interdisciplinary teaching, and provide essential instructions for discharge regarding treatment, follow-up, and medications.

The Must-Haves
• Background: Graduation from an accredited school of nursing; Early Career.
• Experience: Must possess temporary authorization to practice as a Graduate Nurse under direct registered nurse supervision, and obtain Michigan RN licensure within 90 days of hire.
• Skills: Clinical judgment and analytical skills for comprehensive care planning; proficient reading, writing, and comprehension skills; mathematical ability to perform calculations; high level of interpersonal and verbal communication for effective teamwork.
• Bonus: BSN preferred.
