This is an early career opportunity at SSM Health Rehabilitation Hospital, located in Lake St. Louis, MO. The hospital is a joint venture dedicated to providing exceptional and compassionate inpatient rehabilitation care for individuals recovering from various complex conditions, including brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas. This role is vital in collaborating with medical staff to ensure comprehensive and effective care plans, directly impacting patients’ recovery and quality of life.

• Mission: To collaborate on, implement, assess, evaluate, and follow-up on individualized patient care plans.
What You’ll Actually Do
• Patient Assessment: Initiate an ongoing systematic assessment of the physical, emotional, social, educational, and functional needs of patients and their families.
• Care Plan Development: Interpret assessment data and information to develop a comprehensive nursing care plan, including problem identification, nursing diagnosis, nursing orders, approaches, interventions, short/long-term goals, and discharge planning.
• Interdisciplinary Collaboration: Actively participate as a productive member of the interdisciplinary team in the assessment, planning, implementation, and evaluation of rehabilitation goals.
• Documentation: Ensure all documentation in the medical record is complete, factual, accurate, and timely.
The Must-Haves
• Background: Graduate of an accredited program for registered nursing.
• Experience: Valid State RN License is required upon start date; BLS (Basic Life Support) certification required at hire.
• Skills: Flexibility in scheduling to complete the requirements of the program.
