• Mission: Plan, develop, organize, evaluate, and direct the Social Services Department to ensure residents receive the highest level of psychosocial support and that their social, emotional, and discharge planning needs are met.

What You’ll Actually Do
• Department Leadership: Plan, develop, organize, evaluate, and direct the overall operation of the Social Services Department in accordance with all current federal, state, and local standards, guidelines, and regulations.
• Resident Care Planning: Assess the social and emotional needs of new and existing residents, then develop individualized care plans in coordination with interdisciplinary teams, while advocating for their rights.
• Advocacy & Family Support: Support residents and families during admissions, transitions, care conferences, and discharge planning, serving as a key liaison between residents, families, facility staff, and community agencies.
• Resource & Program Coordination: Coordinate essential services for residents, such as counseling, legal aid, and financial assistance as needed, and organize support groups or educational programs for residents and families.
• Documentation & Compliance: Document social histories, assessments, progress notes, and care plans meticulously, ensuring strict compliance with Medicare/Medicaid regulations, HIPAA, and state long-term care requirements, and participate in facility surveys by assisting in plans of correction.

The Must-Haves
• Background: Bachelor’s degree in Social Work (BSW) and a current Social Work license or certification as required by state regulations.
• Experience: Minimum of 2 years’ experience in a long-term care facility or a similar healthcare setting.
• Skills: Strong understanding of long-term care regulations, discharge planning, resident advocacy, excellent communication, interpersonal, and organizational skills.
• Bonus: Master’s degree (MSW) is preferred.
