The Tone:
This is an in-house Director of Rehabilitation role at Evolve Therapy Services, LLC, located in Franklin, MN. Evolve Therapy Services is a therapy management organization dedicated to hiring compassionate therapists for our partner Skilled Nursing Facilities. This role is critical as it oversees all aspects of therapy operations, ensuring the provision of the best possible therapy services to patients consistent with established standards and company procedures.

• Mission: Owns the oversight and management of all therapy program aspects to provide high-quality services in a Skilled Nursing Facility.

What You’ll Actually Do
• Program Oversight: Provide oversight and management for all aspects of the therapy program in the assigned facility or facilities.
• Compliance Assurance: Ensure knowledge of and adherence to all regulatory requirements, company policies, procedures, and HIPAA and confidentiality standards.
• Procedure Development: Establish facility-specific therapy procedures in collaboration with the interdisciplinary team and provide training to all therapy staff on these procedures.
• Therapist Supervision: Ensure all therapists provide services and adhere to documentation and billing practices in accordance with regulatory requirements, standards of practice, and company procedures.
• Communication: Communicate and interact professionally and respectfully with patients and family members to attend to and meet their requests.

The Must-Haves
• Background: Degree in Physical Therapy, Occupational Therapy, or Speech Language Pathology; this is a Director-level position.
• Experience: Must have an active PT, PTA, OT, OTA, or SLP license in the state(s) of practice.
• Skills: Demonstrates excellent verbal, written, and interpersonal leadership and communication skills, alongside strong critical thinking and problem-solving abilities.
• Bonus: Experience with PDPM and MDS management is preferred.
