The Tone:
This is a full-time Director of Rehabilitation role with Evolve Therapy Services, LLC, situated in Franklin, MN. Evolve Therapy Services is a dedicated therapy management organization focused on recruiting compassionate therapists for its partner skilled nursing facility. This pivotal position is responsible for overseeing and managing all therapy operations within the facility, ensuring the provision of the best possible therapy services to patients. The role is instrumental in maintaining established standards of practice and upholding company and department procedures, contributing significantly to patient well-being and facility compliance.

• Mission: Owns all aspects of therapy operations at the facility to ensure quality patient care and compliance.

What You’ll Actually Do
• Therapy Program Management: Provide comprehensive oversight and management for all aspects of the therapy program within the assigned facility, ensuring the delivery of best possible therapy services to patients consistent with established standards of practice, company policies, and department procedures.
• Regulatory & Policy Adherence: Ensure full knowledge of and adherence to all regulatory requirements, company policies, procedures, processes, guidelines, job descriptions, certification requirements, HIPAA, confidentiality standards, and resident rights across all therapy operations.
• Procedure Establishment & Training: Collaborate with the facility’s interdisciplinary team to establish facility-specific therapy procedures, and then effectively train all therapy staff on these established procedures to ensure consistent application.
• Therapist Performance & Standards: Verify that all therapists deliver therapy services in strict accordance with regulatory requirements, recognized standards of practice, Company policies and procedures, and therapy department productivity standards, while also ensuring adherence to documentation and billing procedures.
• Interpersonal & Operational Duties: Communicate and interact professionally and respectfully with patients and family members to meet their requests, and perform all other duties as assigned to effectively discharge responsibilities and act in the best interests of the organization.

The Must-Haves
• Background: Hold a degree in Physical Therapy, Occupational Therapy, or Speech Language Pathology. This is a Director-level position overseeing comprehensive therapy operations.
• Experience: Must possess an active license as a Physical Therapist (PT), Physical Therapist Assistant (PTA), Occupational Therapist (OT), Occupational Therapist Assistant (OTA), or Speech Language Pathologist (SLP) in the state(s) of practice. Requires ongoing continuing education and professional development to maintain current licensure and certification.
• Skills: Demonstrate excellent verbal, written, and interpersonal leadership and communication skills. Must be results-oriented with strong critical thinking and problem-solving abilities to navigate complex operational challenges.
• Bonus: Prior experience with PDPM (Patient Driven Payment Model) and MDS (Minimum Data Set) management is preferred for this role.
