Director of Clinical Transition | Saber Healthcare Group
The Tone:
This is a full-time, travel-based role across multiple states for Saber Healthcare Group. Saber Healthcare Group is a company dedicated to senior care, expanding its reach through new acquisitions. This role is crucial for successfully integrating newly acquired clinical facilities, ensuring they adopt company standards, comply with regulations, and maintain high-quality patient care.

• Mission: Guide newly acquired facilities through clinical transition, ensure adherence to company standards, and maintain full compliance with all regulatory requirements.
What You’ll Actually Do
• Policy Implementation: Lead the implementation and continuous monitoring of current clinical services policies and procedures across all assigned newly acquired communities, ensuring operational consistency and alignment with company standards.
• Regulatory Compliance: Guarantee that all assigned communities consistently meet and exceed compliance standards set by state, federal, and other relevant government agencies through ongoing oversight and support.
• Survey Management: Execute regular compliance surveys at assigned communities and actively participate in any state and federal survey activity as necessary, ensuring a smooth and compliant process.
• Clinical Oversight: Conduct regular site visits to provide direct supervision, offer expert clinical guidance, and continuously monitor the overall operation of clinical services within each newly acquired community.
• Staff Development: Develop and deliver in-service training and comprehensive orientation programs to facility staff, focusing on survey processes, risk management, community compliance, and performance improvement initiatives.
• Issue Resolution: Review and thoroughly investigate resident complaints as directed, subsequently developing comprehensive Plans of Corrective Action in collaboration with facility Administrators and Directors of Nursing.
The Must-Haves
• Background: Active, unencumbered Registered Nurse (RN) license.
• Experience: At least two (2) years of experience as a Director of Nursing (DON) or Regional Nurse.
• Skills: Clinical Policy Implementation, Regulatory Compliance Monitoring, Staff Training & Development, Clinical Supervision, Corrective Action Planning.
• Bonus: Willingness to travel extensively to facilities located in multiple states based on new acquisition needs.
