The Tone:
This is a full-time, in-person role at Aventura at Creekside, an organization dedicated to providing skilled nursing and rehabilitation care. This position is crucial for leading the facility’s admissions efforts and for strengthening essential partnerships within the healthcare community. The Admissions Director / Hospital Liaison helps ensure patients experience a seamless transition into skilled nursing and rehabilitation care, directly contributing to the facility’s mission and growth. This role requires an individual who thrives in building relationships and navigating a busy environment.

What You’ll Actually Do
• Relationship Development: Actively develop and consistently maintain strong referral relationships with key hospitals, physicians, case managers, and other crucial healthcare partners to secure a steady stream of patient referrals.
• Primary Liaison: Serve as the direct and primary liaison, fostering effective communication and understanding between external referral sources and the Aventura at Creekside facility.
• End-to-End Admissions Management: Take full ownership of coordinating and managing the entire admissions process, meticulously guiding it from the initial referral contact all the way through to successful patient admission.
• Referral Evaluation and Acceptance: Critically evaluate all incoming patient referrals, ensuring they meet both the appropriate clinical criteria for care and the necessary financial acceptance guidelines of the facility.
• Community and Growth Outreach: Regularly conduct targeted visits to hospitals and other community referral sources to actively generate new referral opportunities, while also tracking all admissions activity, referral trends, and progress towards established census goals.
• Interdisciplinary Collaboration: Collaborate closely with the facility’s interdisciplinary team to meticulously plan and execute smooth and effective transitions for residents entering care.
• Marketing and Networking: Actively participate in various marketing initiatives, networking events, and community outreach programs to enhance the facility’s presence and referral pipeline.

The Must-Haves
• Background: A Director-level professional with extensive experience in healthcare admissions, business development, hospital liaison functions, or clinical marketing roles within the healthcare sector.
• Experience: Requires previous, demonstrated experience in healthcare admissions or related business development, along with a valid driver’s license and reliable transportation for frequent local travel to referral sources.
• Skills: Possesses excellent communication, relationship-building, and negotiation skills, alongside strong organizational and time management abilities. Must be capable of multitasking and working independently in a high-volume environment, with proficiency in Microsoft Office and electronic medical record systems.
• Bonus: Strong preference for candidates who bring specific experience from a Skilled Nursing Facility (SNF), rehabilitation center, long-term care facility, or acute hospital setting.
