Office of Mental Health Case Manager | Odyssey House

The Tone:
This is a full-time role at Odyssey House, located in New York, NY. Since 1967, Odyssey House has been providing innovative services and programs to individuals and families struggling with substance use and mental health disorders across all five boroughs. This role is crucial for delivering highly individualized treatment programs, supporting personal rehabilitation, renewal, and family restoration. The Case Manager is vital in stabilizing individuals and promoting self-sufficiency and optimum health for people living with HIV/AIDS, mental illness, and/or substance use disorders.

• Mission: Stabilize and promote self-sufficiency and optimum health for people living with HIV/AIDS, Mental illness, and/or substance use disorders.

What You’ll Actually Do
• Provide comprehensive substance abuse intervention, counseling, educational services, and case management to tenants within the program.
• Evaluate tenants through one-to-one interviews to recommend and develop appropriate treatment plans for those with mental health and chemical dependency problems.
• Manage a dedicated caseload of 20 to 25 individuals, offering advocacy and coordinating essential services, including through regular home visits.
• Maintain meticulous and up-to-date tenant charts, detailed case notes, service plans, and psychosocial documentation, and present the assigned caseload weekly at chart review.
• Establish and nurture strong community and agency affiliations to strengthen the delivery of services for tenants, including medical and detoxification when necessary, and oversee the collection of tenants’ monthly rent.

The Must-Haves
• Background: Entry-Level professional with a High School Diploma and demonstrated experience, or a Bachelor’s Degree in Human Service.
• Experience: One to two years of experience in counseling, or two (2) years of experience specifically within Odyssey House.
• Skills: Substance abuse counseling and intervention, case management, treatment plan evaluation and recommendation, record keeping and documentation, and coordinating services for individuals with complex needs.
