Director of Rehab (DOR) In House SNF (5k SO) | Evolve Therapy Services, LLC

This is a leadership role with Evolve Therapy Services, LLC, located in Franklin, MN. Evolve Therapy Services is a therapy management organization dedicated to hiring compassionate therapists for its partner facilities. This role is crucial for ensuring the highest quality therapy operations and patient care within a Skilled Nursing Facility, driving compliance and service excellence.

The TL;DR
• Role: Full Time
• Location: In-person, Franklin, MN
• Team: Reports to the Regional Director of Operations
• Mission: Responsible for all aspects of therapy operations at the facility in accordance with established policies, procedures, and practices.

What You’ll Actually Do
• [Leadership]: Oversee and manage all aspects of the therapy program in the assigned facility or facilities.
• [Compliance]: Ensure knowledge of and adherence to all regulatory requirements, company policies, procedures, and resident rights.
• [Program Development]: Establish facility-specific therapy procedures in collaboration with the interdisciplinary team and train all therapy staff on these procedures.
• [Service Excellence]: Guarantee therapists provide therapy services consistent with established standards of practice, company policies, and productivity standards.
• [Financial Oversight]: Ensure all therapists adhere to regulatory requirements and Company procedures regarding documentation and billing of therapy services.

The Must-Haves
• Background: Requires a degree in Physical Therapy, Occupational Therapy, or Speech Language Pathology for this leadership position.
• Experience: Must hold an active license as a PT, PTA, OT, OTA, or SLP in the state(s) of practice, coupled with ongoing continuing education for licensure and certification.
• Skills: Demonstrates excellent verbal, written, and interpersonal leadership and communication skills, alongside strong critical thinking, problem-solving abilities, and a results-oriented approach.
• Bonus: Experience with PDPM and MDS management is preferred.
