Director of Rehab (DOR) | Evolve Therapy Services, LLC
The Tone:
This is a full-time role at Evolve Therapy Services, LLC, located in Franklin, MN. Evolve Therapy Services is a therapy management organization hiring compassionate therapists for its partner facilities. This critical role leads all therapy operations within an in-house Skilled Nursing Facility. You will ensure the delivery of high-quality therapy services that adhere to established policies, procedures, and professional standards.

• Mission: To oversee and manage all therapy operations within an in-house Skilled Nursing Facility, ensuring services meet regulatory and company standards.
What You’ll Actually Do
• Manage: Oversee and manage all aspects of the therapy program within the assigned facility.
• Develop: Establish facility-specific therapy procedures in collaboration with the interdisciplinary team and train all therapy staff.
• Ensure Compliance: Verify adherence to all regulatory requirements, company policies, and standards regarding therapy services, documentation, and billing.
• Oversee Services: Ensure all therapists provide services consistent with established standards of practice, company procedures, and productivity requirements.
• Communicate: Interact professionally and respectfully with patients and family members to meet their requests and needs.
The Must-Haves
• Background: Degree in Physical Therapy, Occupational Therapy, or Speech Language Pathology, leading therapy operations within a Skilled Nursing Facility.
• Experience: Active licensure as a PT, PTA, OT, OTA, or SLP in the state of practice, with ongoing commitment to continuing education and professional development.
• Skills: Excellent verbal, written, and interpersonal leadership and communication skills, coupled with strong critical thinking and problem-solving abilities.
• Bonus: Experience with PDPM and MDS management is preferred.
