Admissions Coordinator | Williamsburg Landing
The Tone:
This is a full-time role at Williamsburg Landing, located in Virginia, United States. Williamsburg Landing operates a Health & Rehabilitation Center (HRC) that provides essential care and services. This position is pivotal to the HRC’s operational success, as it directly manages all aspects of referral and admission activities. The Admissions Coordinator ensures the facility consistently meets and exceeds its budgeted resident census levels by proactively engaging with external and internal referral sources.

• Mission: Owns the process of managing and driving all referral and admission activities to maintain and exceed the Health & Rehabilitation Center’s budgeted census levels.

What You’ll Actually Do
• Drive Census Growth: Proactively engage daily with discharge planners and case managers to achieve and surpass facility budgeted census targets, including internal collaboration and conducting external visits to potential referrals.
• Manage Admissions Data: Track comprehensive monthly and quarterly admission data, including total admissions, discharges, and volume from specific referral sources, for QAPI committee review, and maintain associated dashboards and spreadsheets.
• Facilitate Resident & Family Experience: Conduct informative tours for prospective admissions, communicate effectively with resident representatives regarding discharge plans, ensure appropriate bed holds are offered, and complete inpatient satisfaction surveys.
• Determine Eligibility & Financials: Review medical information to confirm admissions eligibility, verify benefit and insurance details, perform medication cost analyses, and obtain all necessary prior authorizations for Medicare Advantage and managed care admissions.
• Provide Training & Backup: Train Social Services/Discharge Planners and nursing staff on admissions competencies to ensure efficient processes, and coordinate discharges and other essential functions during absences of the Social Services/Discharge Coordinator.
• Support Operations & Marketing: Coordinate room readiness for new admissions, support marketing team initiatives through meetings and material development, and manage administrative tasks such as completing new admission contracts and updating resident data in various portals.
The Must-Haves
• Background: Bachelor’s degree in a related field is preferred. Candidates must possess current and comprehensive knowledge of Medicare and managed care regulations.
• Experience: Demonstrated relevant experience in post-acute care admissions.
• Skills: Exhibits strong initiative and sound judgment in decision-making. Possesses effective communication skills for engaging with diverse stakeholders. Capable of meticulous data tracking and reporting.
