The Tone:

• Mission: To provide a full range of delegated, interdependent, and independent nursing services to patients, ensuring quality outcomes through the nursing process.

What You’ll Actually Do
• Coordinate Comprehensive Care: Coordinate the development of multi-disciplinary plans of care in accordance with Nursing Problem Care Sets, clinical practice guidelines, age-specific, professional, and regulatory requirements to ensure appropriate length of stay, resource use, and quality outcomes. This includes involving the patient, family, and significant others in the plan and collaborating for effective discharge planning with community agencies.
• Perform Therapeutic Regimens: Administer medications, IVs, and blood products according to established policies, performing basic nursing procedures such as dressing changes, catheterization, NG tube insertion, and suctioning. This also involves establishing and maintaining peripheral IV therapy, obtaining body fluid and blood specimens, and performing blood capillary glucose monitoring.
• Monitor and Respond to Patient Changes: Regularly review work in progress and patient medical records, monitoring patient physiologic parameters including vital signs, lab work, I&O, and blood glucose. Recognize acute changes in respiratory, neurologic, or cardiac status, decreased LOC, or urinary output, and respond appropriately with direct intervention, physician, or SWAT notification, utilizing tools like SBAR.
• Educate and Support Patients/Families: Assess patient/family needs for information and education across the continuum, planning and implementing patient teaching using various techniques and methods. This involves explaining medications and treatments, reviewing discharge plans, including signs/symptoms to watch for, and ensuring appropriate follow-up is arranged.
• Ensure Quality and Safe Practices: Promote a physically safe environment for patients, including the appropriate use and documentation of restraints and universal precautions. Identify ethical issues, adhere to patient confidentiality, make cost-effective use of supplies, and actively participate in quality monitoring, improvement activities, and the teaching/mentoring of student nurses and orientees.

The Must-Haves
• Background: Graduate of an approved professional (RN) nursing program, applying core domain knowledge in the nursing process, Seven Dimensions of Patient Care, and Benner’s Domains of Nursing Practice.
• Experience: Must obtain a Michigan Board of Nursing Registered Nurse license within 2 months of hire and American Heart Association BLS Provider certification within 3 months of hire; the position requires approximately 3 to 9 months of on-the-job experience to fully apply unit policies and standards.
• Skills: Demonstrated independent professional judgment, strong analytical skills for problem resolution, clinical competence, effective interpersonal and communication abilities, and the capacity to respond appropriately to life-threatening emergencies.
