MSW Intern | Phamily
The Tone:
This is an internship at Phamily, located in New Lenox, IL, with 5 hybrid days within a 30-minute radius. Jaan Health, operating as Phamily, is an AI-based care management company that uses proprietary technology to help healthcare providers deliver high-quality proactive care. The company’s Phamily platform transforms chronic disease management, enabling physicians and care teams to offer individualized patient care, reduce investment in extra labor, and improve patient lives. This role is crucial for developing and launching a new Behavioral Health Integration program for Silver Cross Medical Group, bridging the gap between physical medicine and mental health, and empowering patients to achieve their health goals.

• Mission: Develop and launch a new Behavioral Health Integration program, providing patient-centered, short-term brief therapy to bridge physical and mental health.

What You’ll Actually Do
• Co-Develop BHI Program: Partner with Phamily’s Lead LCSW to design workflows, build resource networks, and establish clinical processes for the new behavioral health initiative.
• Drive Patient Enrollment: Grow patient enrollment into Phamily’s Care Management services by delivering social work support, especially focusing on historically marginalized communities.
• Conduct Assessments: Meet with patients in-office or via telehealth to complete comprehensive Bio-Psycho-Social Assessments and Social Determinants of Health (SDoH) screenings.
• Provide Evidence-Based Brief Therapy: Deliver client-centered, short-term brief therapy focused on immediate goals, using modalities like Motivational Interviewing, Solution-Focused Brief Therapy, and Cognitive Behavioral Therapy.
• Support Care Management Escalations: Act as a specialized resource for the Care Management team, addressing complex behavioral or mental health needs outside traditional care managers’ scope.
The Must-Haves
• Background: Actively enrolled in an accredited Master of Social Work (MSW) program, tracking toward a clinical or micro-practice concentration.
• Experience: Interest in helping build a clinical program from scratch, engaging in research and development, and acting as a reliable peer-consultant for care managers.
• Skills: Comfort learning and navigating digital health platforms, collaborating via Slack/Zoom, conducting virtual patient care, strong active listening, clinical boundary skills, empathy, communication, and adaptability to a hybrid work structure.
• Bonus: Entrepreneurial DNA with a ‘builder’ mentality, ability to operate effectively in environments where processes may not be fully fleshed out, bi-lingual in English and Spanish, strong analytical, organizational, problem-solving, leadership, initiative, and collaboration skills.
