In this role, the LPN is integral to providing direct and/or indirect patient care, supporting a team-based approach that emphasizes patient self-management and population health management within physician practices. The TL;DR
• Role: Full Time
• Type: Regular, Standard Hours
• Location: Chattanooga, TN (In-person)
• Team: Physician Practices department, under the direct supervision of a Physician or Registered Nurse (RN).
• Mission: Provides direct and/or indirect patient care, coordinates patient care, and supports patient self-management within a team-based physician practice environment.

What You’ll Actually Do
• Patient Care: Provide direct and/or indirect patient care under the direct supervision of a Physician or Registered Nurse, adhering to State of Tennessee/Georgia guidelines.
• Care Coordination: Coordinate care for patients, their families, and caregivers, supporting their self-management, self-efficacy, and behavior change.
• Population Management: Manage the practice’s patient population, ensuring comprehensive and consistent care delivery.
• Quality Improvement: Actively participate in the practice’s quality improvement processes and performance evaluations to enhance patient outcomes.
• Medical Administration: Administer pharmacological drugs with a thorough understanding of their properties and perform necessary procedures and operate equipment for patient care as assigned.

The Must-Haves
• Background: Graduate from an accredited School of Practical Nursing, with foundational knowledge in general nursing principles and practice skills.
• Experience: Demonstrated knowledge of general nursing principles, organizational functions, policies, regulations, and current trends in nursing. Proven ability to work effectively with people, exhibiting a poised, courteous, and pleasing personality, alongside professional public interaction skills including a good telephone voice.
• Skills: Patient Assessment & Documentation according to unit standards, Pharmacology administration and interaction, Age-Appropriate Care, Interpersonal Communication, Implementing and evaluating individualized patient care plans.
• Bonus: PALS (Pediatric Advanced Life Support) certification, previous pediatric experience, and practical nursing licensure in the State of Tennessee are preferred.
