Housing Support Case Manager | Claratel Behavioral Health
The Tone:
This is a full-time, community-based position at Claratel Behavioral Health, located primarily in DeKalb County, Georgia, with required travel throughout DBHDD Region 3. Claratel Behavioral Health is a public, not-for-profit organization that provides a comprehensive range of mental health, developmental disabilities, and substance use disorder services to underserved individuals, operating in more than 20 locations with over 400 direct-care and support staff. This Housing Support Case Manager role is integral to a program designed to promote housing stability, wellness, independence, recovery, and community integration for adults living in permanent subsidized housing. Your work in this role directly supports individuals by mitigating factors that impact housing stability and fostering resilience, contributing to a healthier future for all.

What You’ll Actually Do
• Client Caseload Management: Carry a caseload of 25-30 clients, conducting a minimum of one weekly visit per client for the first three months after obtaining housing, then twice per month thereafter, within their residence or community setting.
• Housing Stability Support: Provide comprehensive assistance with housing search, leasing, and move-in processes, including purchasing initial furnishings, deposits, and household goods for one-time move-in needs.
• Care Coordination and Linkage: Provide care coordination to ensure housing stability and independent living skills, facilitating referrals and linkages to internal and external community services, general entitlements, psychiatric/substance use/medical services, and crisis prevention and intervention.
• Early Intervention and Education: Implement early intervention strategies to mitigate factors impacting housing stability, such as late rent payment, lease violations, or tenant/landlord conflicts, and educate tenants on their roles, responsibilities, and rights.
• Documentation and Team Collaboration: Complete timely and high-quality documentation for all service contacts within 24 hours to meet agency and state standards, and participate in weekly team meetings to provide input for client treatment needs.
The Must-Haves
• Background: Bachelor’s degree in a related field such as human services, psychology, counseling, criminal justice, sociology, or social work, or equivalent education and experience, preparing for a case management role.
• Experience: One year of experience providing case management or community support services in a behavioral health setting.
• Skills: Proficiency in using electronic medical records, computers, telehealth systems, and cell phones; strong care coordination abilities; effective interpersonal skills; a positive team-player attitude; and a valid Georgia driver’s license.
