ECM Supervisor | Elica Health Centers
The Tone:
This is a full-time role at Elica Health Centers, located in North Highlands, CA. Elica Health Centers is a healthcare organization dedicated to providing exceptional patient care and vital community resources to underserved populations through its clinics, mobile medicine program, and resource center. This role is crucial for leading the Enhanced Care Management (ECM) program, which extends beyond traditional healthcare settings to coordinate comprehensive health and social services for individuals with complex needs, ensuring they receive personalized support wherever they are.

• Team: Reports to the CalAIM Manager, supervises the ECM team.
• Mission: Supervise and manage daily operations of the ECM team to ensure compliance with program requirements and successful delivery of care coordination services to clients with complex needs.
• Tech Stack: EPIC, Google Workspace, Microsoft Excel
What You’ll Actually Do
• Oversee program compliance: Supervise employees’ adherence to CalAIM ECM and Managed Care Plans (MCPs) requirements, providing guidance and addressing non-compliance.
• Manage team operations: Lead the ECM team’s day-to-day activities, ensuring successful completion of program requirements and timely service delivery.
• Guide care planning: Collaborate with the ECM Clinical Consultant to oversee the development and implementation of patient-centered care plans by ECM Care Managers.
• Provide staff development: Onboard and train new team members, identify development needs, and provide ongoing mentorship and performance evaluation for ECM Care Managers.
• Monitor quality and reporting: Conduct regular internal audits, approve documentation, monitor quality oversight reports, and prepare periodic reports for management.
The Must-Haves
• Background: Minimum of a Bachelor’s degree. Career level: Supervisor. Core domain knowledge includes Care Management, the ECM program, HIPAA regulations, and medical terminology.
• Experience: A minimum of three years of Care Management experience is required, along with prior experience in a supervisory or team lead role. Experience working with individuals experiencing homelessness, chronic illness, substance use disorders, and serious mental illness is also necessary.
• Skills: Strong understanding of the ECM Program and its documentation requirements, proficiency with EPIC and Client Centered Care Planning, excellent computer skills including Microsoft Excel and Google Workspace, and outstanding interpersonal relationship building and communication skills.
• Bonus: Experience in outreach and inter-agency referral services, knowledge of various Managed Health Plans (MHPs) and state programs, and experience in a team-oriented workplace are all preferred.
