• Team: Works within a supportive care team, reporting to a supervisor.
• Mission: Provide personalized care and supportive services to residents, enhancing their quality of life and contributing to facility goals.

What You’ll Actually Do
• Provide direct care: Offer medication reminders and comprehensive personal assistance, including showering.
• Maintain living spaces: Handle laundry, perform linen changes, and deliver homemaking services.
• Ensure mobility and access: Escort residents to appointments and activities, providing transportation services as needed.
• Support social well-being: Offer companion and sitter services, fostering positive resident interactions.
• Implement care plans: Execute related medical activities and supportive services as outlined in individual care plans.

The Must-Haves
• Background: A strong positive attitude towards customer service and a genuine interest in geriatric care, coupled with the ability to motivate and maintain positive relationships with both staff and residents.
• Experience: Proven ability to provide various personal care services as outlined on care plans, serving multiple clients throughout the day at scheduled times. This position requires travel to meet patient needs.
• Skills:
• Adherence to safety needs of patients and proper equipment usage (e.g., foot stools, side rails).
• Strict compliance with all organizational privacy policies, infection control protocols, and confidentiality of Protected Health Information (PHI).
• Consistent demonstration of best effort, creativity, and commitment to accomplishing facility goals.
• Active participation in assigned in-service training and education sessions.
• Execution of specific work duties and responsibilities as assigned by supervisor to ensure quality, continuity, and safety of care.
