RN Transitional Care Coordinator | AMN Healthcare Nursing

The Tone:
This is a travel nursing role with AMN Healthcare Nursing, located in Salinas, California. AMN Healthcare is a leader in nurse staffing, connecting healthcare professionals with numerous facilities across the country. This role is crucial for ensuring smooth transitions of care for patients being discharged, focusing on reducing hospital readmissions, improving patient outcomes, and enhancing patient satisfaction.

• Pay: $1800–$2700 weekly
• Mission: Optimize recommendations focusing on reducing hospital readmissions, improving patient outcomes, and enhancing patient satisfaction for discharged patients.

What You’ll Actually Do
• Transitions: Ensure smooth transitions of care for patients being discharged from the hospital setting.
• Optimization: Work with population specific, clinic specific, insurance specific, high-risk, or virtual home program patients to optimize recommendations focusing on reducing hospital readmissions, improving patient outcomes, and enhancing patient satisfaction.
• Planning: Assess, evaluate, and implement a plan of care for the patient in collaboration with the physician, hospital care teams, and family/significant others.
• Coordination: Work collaboratively with the Transitional Care Program Leadership team, Transitional Care Social Worker, Case Management staff, and other multidisciplinary team members to develop a Continuum of Care plan.
• Follow-up: Follow up with the patient and their support structure to ensure compliance with the medical treatment plans.

The Must-Haves
• Background: Associate Degree in nursing with a broad general knowledge of nursing, along with a working knowledge of community resources, post-acute care coordination, and case management principles.
• Experience: Requires 3 years’ nursing experience and recent experience with Epic.
• Skills: Ability to effectively navigate and utilize a computerized medical record system. Possesses a current CA RN license and AHA BLS certification.
• Bonus: Bachelor of Science in Nursing (BSN), Case Management experience, or bilingual proficiency in Spanish.
