About the Job: Physician/Medical Director

Theoria Medical is seeking dedicated Physicians to join its team, specifically for roles as Medical Directors and Attending Physicians within the skilled nursing facilities (SNFs) it manages. This position offers a part-time, exempt status with flexible scheduling options tailored to fit individual provider needs, within a positive work environment.

Job Location: This is an in-person position at the assigned facilities.

Essential Functions & Responsibilities:

The physician in this role will primarily serve in a dual capacity:

1. Medical Director for Skilled Nursing Facilities (SNF):
Participate in administrative and quality initiatives, including attending and participating in monthly quality assurance and process improvement (QA/PI) meetings.
• Engage in quality improvement initiatives.
• Provide guidance to facility staff.
• Review and revise (if necessary) facility’s clinical guidelines.
• Ensure compliance with state and federal regulations.
• Train and supervise facility clinical staff.

2. Attending Physician for SNF Residents:
• Collaborate closely with Nurse Practitioners and Physician Assistants to manage the day-to-day resident care. Each facility typically has a full-time PA or NP assigned based on patient acuity and volume.
• Provide high-quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs.
• Responsible for diagnosing and treating patients with acute illnesses (e.g., bacterial and viral infections) as well as chronic diseases and conditions (e.g., hypertension, heart disease, diabetes).
Documentation is completed in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
• Familiarity with other commonly used facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech is beneficial.
• Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring (RPM), and other medical quality initiatives and programs.
Supervise collaborating nurse practitioners and physician assistants.
• Utilize electronic/online medical tools such as EHR systems and state/county medical systems for Death Certificates and administrative functions.

Shift Structure:

Flexible shifts depending on the physician’s schedule.
• Typically, medical directors work a minimum of 2-3 days per week at each facility they oversee.
• Rounding times are flexible with no set start and end time, preferably starting in the morning between 7-10 a.m. and finishing when the caseload has been seen.
Call is covered by Theoria’s virtual care team, ensuring no overnight duties for the physician.
• This is an in-person position.

Compensation and Benefits:

Competitive compensation package, which includes a Medical Director stipend.
• Compensation ranges approximately from $50,000-$400,000/year, depending upon employment status and availability (PRN, Part-Time, Full-Time).
