Job Description: Licensed Practical Nurse, Home Health

As a Licensed Practical Nurse (LPN) in Home Health at CenterWell, you will be an integral part of a caring community, providing personalized, one-on-one nursing care to patients in the comfort of their homes. This role is crucial in supporting patient recovery, fostering independence, and making a meaningful impact daily. You will work closely with a dedicated team of physicians and clinicians, developing and managing care plans to help patients regain their independence and quality of life.

Key Responsibilities:

Provide Professional Nursing Services: Deliver skilled nursing interventions under the direct supervision of a Registered Nurse (RN). All care must be in compliance with the state’s Nurse Practice Act, applicable licensure/certification requirements, healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
Skilled Interventions & Holistic Care: Perform skilled nursing interventions for the treatment of patient illnesses, rehabilitative needs, and preventative care. Utilize a holistic approach in providing problem-specific therapeutic interventions, along with teaching and training activities, all in accordance with the established plan of care.
Clinical Application: Apply knowledge and skills in line with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
Care Plan Management & Collaboration: Confer with the RN Supervisor regarding any necessary changes in the Plan of Treatment. Accept verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act, ensuring these orders are promptly communicated to the RN Supervisor.
Goal-Oriented Approach: Implement the nursing plan of care using a systematic, individualized, and goal/outcome-driven approach.
Communication & Coordination: Maintain timely contact with patients, physicians, clinical manager(s), and other members of the healthcare team regarding patient/family needs and any status changes. Participate actively in care coordination activities and discharge planning as appropriate.
Confidentiality: Uphold the highest standards of professional conduct regarding confidential information, sharing it only when the recipient’s right to access is clearly established and the sharing is in the patient’s best interests.
Resource Management: Communicate appropriately to ensure adherence to professional standards regarding the provision and availability of supplies, materials, and equipment necessary for the safe and effective implementation of the plan of care.
Documentation: Prepare, submit, and maintain all required documentation as per company and/or facility guidelines. Visit/shift notes must be documented on the same day services are rendered.

Required Experience/Skills:

Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Current nursing license in the practicing state.
Transportation: Valid driver’s license, auto insurance, and reliable transportation are essential.
Certifications: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, with a strong preference for experience in a home health or hospice setting.

Compensation & Hours:

Scheduled Weekly Hours: 40 hours.
Pay Range (Annual Base Pay): $44,600 – $61,400 per year for full-time employment (40 hours per week). This range reflects a good faith estimate of starting base pay and may vary based on geographic location and individual factors such as demonstrated job-related skills, knowledge, experience, education, and certifications.
Alternative Pay (Per Visit/Unit): $28.00 – $40.00 per visit/unit.
