This role is about joining a caring community to help prioritize health and make a meaningful daily impact as a CenterWell Home Health nurse. You will provide personalized, one-on-one care to help patients regain independence in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you will develop and manage care plans that support patient recovery and help them return to their desired lives.
As a Home Health LPN, You Will:
• Provide professional nursing services under the direction of a Registered Nurse (RN), ensuring compliance with the state’s Nurse Practice Act, applicable licensure/certification requirements, healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
• Deliver skilled nursing interventions for patients’ illness treatment, rehabilitative needs, and preventative care. This involves utilizing a holistic approach for problem-specific therapeutic interventions, as well as teaching and training activities, all in accordance with the established plan of care.
• Apply knowledge and skills in line with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Consult with the RN Supervisor regarding any necessary changes in the Plan of Treatment. You will also accept verbal orders from physicians where permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.
• Implement the nursing plan of care using a systematic, individualized, goal/outcome-driven approach.
• Maintain timely contact with patients, physicians, clinical manager(s), and other members of the healthcare team regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
• Uphold the highest standards of professional conduct concerning confidential information. Information should only be shared when the recipient’s right to access is clearly established, and the sharing of such information is demonstrably in the best interests of the patient.
• Communicate appropriately to ensure adherence to professional standards regarding the provision and availability of supplies, materials, and equipment needed to safely and effectively implement the plan of care.
• Prepare, submit, and maintain documentation as required by the Company and/or facility. Visit/shift notes must be documented on the day services are rendered.
Required Experience/Skills:
• Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Licensure: Current nursing license in the practicing state.
• Transportation: Valid driver’s license, auto insurance, and reliable transportation.
• Certifications: Current CPR certification.
• Experience: Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
Pay Range and Scheduled Weekly Hours:
• Pay per visit/unit: $28.00 – $40.00
• Annual base pay: $44,600 – $61,400 per year. This range reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. Individual pay may vary based on geographic location and demonstrated job-related skills, knowledge, experience, education, and certifications.
