Job Description

This role is for a Home Health LPN within CenterWell Home Health, focusing on providing one-on-one personalized care to patients in their homes to help them regain independence.

Role and Responsibilities:
• Provide professional nursing services under the direction of a Registered Nurse (RN), adhering to the state’s Nurse Practice Act, licensure/certification requirements, healthcare standards, governmental laws, and CenterWell Home Health policies and procedures.
• Deliver skilled nursing interventions for patient illness, rehabilitative needs, and preventative care.
• Utilize a holistic approach, applying problem-specific therapeutic interventions, teaching, and training activities as per the plan of care.
• Apply clinical practice knowledge and skills to facilitate problem resolution and achieve individualized patient goals.
• Confer with the RN Supervisor regarding necessary changes to the Plan of Treatment.
• Accept verbal orders from physicians where permitted by state law/regulations/Nurse Practice Act and communicate these to the RN Supervisor.
• Implement the nursing plan of care using a systematic, individualized, goal/outcome-driven approach.
• Maintain timely communication with patients, physicians, clinical manager(s), and other healthcare team members regarding patient/family needs and status changes.
• Participate in care coordination activities and discharge planning as appropriate.
• Maintain the highest standards of professional conduct concerning confidential patient information, sharing it only when access is clearly established and beneficial to the patient.
• Ensure adherence to professional standards regarding the availability and provision of necessary supplies, materials, and equipment for safe and effective care implementation.
• Prepare, submit, and maintain required documentation, ensuring visit/shift notes are documented on the day services are rendered.

Required Experience/Skills:
Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Current nursing license in the practicing state.
Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Certifications: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, with a preference for home health or hospice experience.

Work Schedule and Compensation:
Scheduled Weekly Hours: 40 hours.
Pay Range: A good faith estimate of the starting base pay is $56,200 – $77,400 per year. Actual pay may vary based on geographic location and individual qualifications (skills, knowledge, experience, education, certifications).
