Licensed Practical Nurse Home Health at CenterWell Home Health

Summary: This role involves providing skilled nursing care to patients in their homes, aiming to support their recovery and independence. LPNs operate under the supervision of registered nurses and physicians. Key responsibilities include developing and implementing individualized care plans, coordinating with healthcare teams, and ensuring compliance with healthcare laws and standards. The work emphasizes personalized therapeutic interventions, patient education, and thorough documentation to promote overall health and well-being. CenterWell Home Health prioritizes personalized, one-on-one care to help patients regain independence in their homes, working closely with physicians and clinicians to manage care plans that facilitate recovery.

As a Home Health LPN, You Will:
• Provide professional nursing services under the direction of a Registered Nurse (RN), ensuring compliance with the state’s Nurse Practice Act, licensure/certification requirements, healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
• Deliver skilled nursing interventions for patient illness treatment, rehabilitative needs, and preventative care. This involves utilizing a holistic approach, providing problem-specific therapeutic interventions, and conducting teaching/training activities aligned with the patient’s plan of care.
• Apply knowledge and skills in accordance with accepted clinical practice standards to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Confer with the RN Supervisor regarding necessary changes in the Plan of Treatment. Accept verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.
• Implement the nursing plan of care using a systematic, individualized, goal/outcome-driven approach.
• Maintain timely contact with patients, physicians, clinical manager(s), and other healthcare team members regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
• Maintain the highest standards of professional conduct regarding confidential information, sharing only when the recipient’s right to access is clear and it is in the patient’s best interest.
• Communicate appropriately to ensure adherence to professional standards in the provision and availability of supplies, materials, and equipment needed for safe and effective implementation of the care plan.
• Prepare, submit, and maintain documentation as required by the Company and/or facility, with visit/shift notes documented on the day services are rendered.

Required Experience/Skills:
Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Current nursing license in the practicing state.
Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Certification: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.

Scheduled Weekly Hours: 40

Pay Range: The estimated starting base pay for full-time (40 hours per week) employment ranges from $56,200 to $77,400 per year. Actual pay may vary based on geographic location and individual qualifications (job-related skills, knowledge, experience, education, certifications).
