Job Description

Position Title: Physician Assistant (PA) or Nurse Practitioner (NP)

Position Type: Part-Time, Exempt

Job Location: In-person at assigned Skilled Nursing Facilities (SNFs)

Shift Structure:
In-person rounding shifts typically start around 7 AM local time, with some flexibility.
• Shifts are primarily available on Saturdays, Sundays, and major holidays.
• This role specifically covers weekend needs and two holidays per calendar year.
Charting/documentation can be completed off-site.
• This is an in-person position requiring physical presence at the SNF.

Essential Functions and Responsibilities:
Rounding and managing the clinical care of the patient population at the assigned Skilled Nursing Facility (SNF).
Daily rounding and prescribing medications and treatments for residents residing at SNFs.
• Providing high quality, patient-centered, comprehensive primary care services to SNF residents.
• Ensuring timely documentation of all patient care and encounters.
• Being available for phone/secure messaging consultations with facility nursing staff.
• Completing provider documentation in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
• Familiarity with commonly used facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech is beneficial.

Physical Requirements:
• Ability to move frequently and briskly inside a facility to see patients, meet with staff, and attend to needs.
Punctual in-person attendance is required (typically between 7 a.m. and 1 p.m. for rounders).
Continuous operation of app-based EMR/notes system via computer or mobile device (iPad, Phone etc.).
• Ability to ascend and descend stairs within a facility.
• Ability to position oneself to obtain records, tools, medical equipment, or other items that may be located below the knee or above the shoulder.
• Ability to effectively communicate with patients, managers, and facility staff.
• Ability to observe details in close range (within a few feet) to provide accurate medical recommendations, treatment, and timely updates to Managers regarding facility or patient concerns.
• Must be able to lift or move at least 25 pounds.

Requirements and Qualifications:
• Candidate must hold a valid PA/NP license in the state of practice.
• Candidate must possess a valid NP/PA degree from an accredited institution.
• Candidate must have an active DEA license. (New graduates will receive assistance with their DEA application from Theoria).
General Post-Acute/SNF knowledge or experience is preferred.
• Candidate must be familiar with EMR systems and charting practices.
