About Beaumont Home Health and Hospice and the Hospice, General In-patient Nurse-Case Manager Role

This Hospice, General In-patient Nurse-Case Manager position offers the opportunity to make a real difference in the lives of patients and their families during end-of-life care. You’ll provide holistic, family-centered care, anticipating, preventing, and treating undesirable symptoms.

Key Details:

Schedule: Monday – Friday, 12pm – 8pm; weekend rotation, one summer and one winter holiday
Location: Based at Trenton & Dearborn Hospitals
Agency: Beaumont Home Health and Hospice
Sign-on Bonus: $5,000

Responsibilities:

Plan of Care: Implement care to achieve comfortable and safe dying, self-determined life closure, and effective grieving for patient and family.
Family Educator/Advocate: Support the patient’s dignity and spiritual/cultural beliefs; provide support at the time of death, including disposition of medications and funeral home notification.
Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
Communication: Provide referral sources with information on hospice programs and coordinate patient care for optimal transition.
Operations: Attend interdisciplinary team meetings for full team discussions, consultation, and evaluation of patient/family needs.
• Assist in determining the need to change the level of care based upon assessment and collaboration with the Attending Physician or Medical Director.

Requirements:

• Bachelor’s Degree in Nursing preferred or Graduate of Diploma or Associate Degree program in Nursing within the state of employment
• Minimum 3 years inpatient clinical experience, preferably hospice
• Valid driver’s license and auto insurance in your name as a driver
• Capable of all physical demands
