Contract Length:

June – December 2026 (Maternity Leave coverage)

Pay Rate:

$53.55/hr. – $59.50/hr. (equivalent to $54 – $59 per hour)

Role Overview:

We are seeking a compassionate and motivated Occupational Therapist (OT) to provide temporary coverage for a maternity leave at a Union City-based care center. This role focuses on delivering high-quality, patient-centered care to a geriatric and frail population, both on-site and through occasional home visits. This is a hands-on, collaborative role within a multidisciplinary care team, offering strong exposure to holistic patient care (rehab, mental health, dietary, etc.).

Key Responsibilities:

• Provide occupational therapy services to elderly/frail patients.
• Develop and implement individualized, care plan-driven treatment plans.
• Conduct home safety evaluations and patient home visits (approximately 4–5/month).
• Collaborate closely with:
  • Other Occupational Therapists
  • Physical Therapists
  • Cross-functional care teams (e.g., rehab, mental health, dietary)
• Maintain accurate documentation (Epic EMR preferred).
• Support continuity of care during maternity leave coverage.

Must Have Qualifications:

• Active Occupational Therapist (OT) license (California).
• Graduate of an accredited Occupational Therapy program.
• Reliable transportation and a driver’s license (for 4-5 local home visits/month).

Preferred Qualifications:

• Bilingual in English + Mandarin or Cantonese.
• Experience working with geriatric or frail populations (1–2+ years preferred).
