Triage Nurse I – Overnight – CareBridge | Elevance Health

The Tone:
This is a virtual, full-time role at Elevance Health, working specifically within CareBridge Health. CareBridge Health is dedicated to helping individuals in home and community-based settings improve their health, independence, and quality of life through home-care and community-based services. As a Triage Nurse I, you will play a crucial part in this mission by determining the appropriate Care Management program for members and assigning them to the correct care management component, handling cases with limited prior triage experience.

The TL;DR
• Role: Full Time
• Type: Overnight: 10pm – 8am (Central Standard Time); eight (8) 10-hour work shifts in a two-week period, including Saturday and Sunday every other weekend.
• Location: Remote
• Pay: $67200–$115920 yearly
• Mission: Responsible for determining the appropriate Care Management program for members referred through internal and external sources by utilizing department guidelines and established criteria.
• Tech Stack: EMR systems

What You’ll Actually Do
• Assess: Utilize the nursing process to meet individual health needs, applying plan benefits and community resources.
• Educate: Inform members about contracted physicians, facilities, and healthcare providers.
• Collaborate: Partner with medical management and care management associates to identify needs and assign members to appropriate care management programs.
• Refer: Facilitate the selection of appropriate candidates for referral to Care Management (CM) and/or Disease Management (DM), and identify cases for referral to other departments like Quality Improvement (QI) or Special Investigations Unit (SIU).
• Document: Record appropriate clinical information, decisions, and determinations in a timely, accurate, and concise manner.

The Must-Haves
• Background: Associate’s Degree (AS) in nursing, suitable for managing least complex cases with limited or no previous Triage care experience.
• Experience: Minimum of 2 years of acute care clinical experience.
• Skills: Current, unrestricted RN license in AZ, FL, IA, IN, KS, MA, NM, OH, TN, TX, NJ, HI or VA.
• Bonus: Current, active, RN Compact license. Emergency Room and/or Urgent Care experience. Telehealth experience. Experience with EMR systems. Bachelor of Science (BS) in nursing. Participation and/or certification in a managed care or utilization management organization. Ability to understand clinical information and prepare concise summaries. Basic knowledge of medical management and care management processes.
