Job Description: Weekend Home Health Registered Nurse

This role is for a Weekend Home Health Registered Nurse at CenterWell Home Health, where the primary aim is to contribute to a caring community focused on putting health first. The position involves making a significant daily impact by delivering personalized, one-on-one care to patients in the comfort of their homes, assisting them in regaining independence. The nurse will collaborate closely with a dedicated team of physicians and clinicians to develop and manage comprehensive care plans that facilitate patient recovery and help them return to their desired lifestyle.

Key Responsibilities:

• Provide skilled nursing visits for home health patients, including admissions, ongoing case management, and follow-up care, specifically on a Weekend Schedule (Thursday, Friday, Saturday, and Sunday weekly).
• Administer continuous care and case management for each patient, ensuring necessary follow-up as directed by the Clinical Manager.
• Collaborate with physicians to establish the initial plan of treatment based on physician’s orders and the patient’s initial assessment.
• Deliver hands-on care, manage and evaluate the care plan, and educate patients in accordance with physician orders, under the supervision of the Clinical Manager.
• Revise care plans in consultation with the physician, based on ongoing assessments and adherence to policy/regulation.
• Coordinate appropriate care involving various healthcare personnel, such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Continuously report patient care, condition, and progress to the patient’s physician and Clinical Manager.
• Implement the patient care plan in conjunction with the patient and their family to help them achieve optimal resolution of their needs and problems.
• Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions/care plans that reflect the current plan of care. Monitor the appropriate completion of documentation by these workers as part of the supervisory/leadership responsibility.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing all required clinical documentation.
• Prepare comprehensive medical documentation for all patients, including records of case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to ensure optimum patient care.

Required Experience/Skills:

• Possession of a Diploma, Associate, or Bachelor Degree in Nursing.
• A minimum of one year of nursing experience in home health is required.
• Proficiency in OASIS (Outcome and Assessment Information Set) is mandatory.
• Experience with Home Care Home Base software is preferred.
• A current and unrestricted Registered Nurse licensure is essential.
• Current CPR (Cardiopulmonary Resuscitation) certification is required.
• Must possess a valid driver’s license, auto insurance, and reliable transportation.

Pay Range and Schedule:

• The compensation for this role can be structured as pay per visit/unit, ranging from $45.00 to $63.00.
• For full-time employment (40 hours per week), the estimated starting base pay ranges from $70,500 to $96,900 per year.
• The scheduled weekly hours are 40. The pay range provided is an estimate of starting base pay for full-time employment at the time of posting and may vary based on geographic location and individual qualifications, including demonstrated job-related skills, knowledge, experience, education, and certifications.
