The Tone:
This is an in-person Team Nurse role within the Beth Israel Lahey Health system, contributing to a large healthcare organization of over 35,000 professionals. This position plays a critical role in directly impacting people’s lives by delivering high-quality, direct patient care in an outpatient primary care or family medicine practice. The Team Nurse ensures patient safety, effectiveness, and efficiency by supporting and coordinating care within a collaborative, team-based model.

What You’ll Actually Do
• `Clinical Patient Management`: Coordinate and facilitate the quality clinical management for a diverse outpatient population, collaborating closely with providers to identify preventive care needs, manage chronic diseases, and provide general and specific health education. This includes working directly with patients to establish and monitor their self-management goals.
• `Care Coordination and Transitions`: Collaborate with community-based providers to ensure seamless patient care transitions. This involves actively following up on emergency department visits and inpatient discharges, as well as assisting primary care providers with various exams, treatments, and medical procedures as required.
• `Direct Patient Care and Medication Management`: Deliver high-quality direct patient care through scheduled nursing visits, encompassing Annual Wellness Visits (AWVs) and Chronic Care Management (CCM) for BIDHC patients. Responsibilities also include managing INR for Coumadin patients according to standardized protocols, administering vaccines and medications, and providing related patient education.
• `Clinical Documentation and Resource Oversight`: Perform a wide range of nursing activities such as wound care and point-of-care testing. Accurately document all aspects of patient care in the Electronic Health Record, schedule tests and specialty referrals, and monitor abnormal test and imaging results. Additionally, oversee inventory, ordering, and management of clinical supplies and biologic specimens.
• `Staff Development and Compliance`: Provide oversight, delegation, and act as a vital clinical resource for unlicensed staff, including Medical Assistants and Nursing Students. This involves ongoing education and training for practice staff, updating clinical knowledge through continuous learning, and ensuring strict compliance with organizational mandatory annual training and patient health information (PHI) protocols.
The Must-Haves
• `Background`: Graduate from an accredited Nursing Program is required. The role requires a current Registered Nurse (RN) license in Massachusetts and certification in Basic Life Support (BLS).
• `Experience`: Requires 1-3 years of related work experience; however, new graduate RNs will be considered for this position. Candidates should also possess advanced skills with Microsoft applications, including Outlook, Word, Excel, PowerPoint, or Access, and proficiency with other web-based applications pertinent to healthcare.
• `Skills`: Demonstrated strong interpersonal skills and excellent patient communication abilities are essential for effective patient interaction and education. Key technical and clinical skills include telephonic triage and assessment, comprehensive patient education, coordination of care, accurate clinical documentation in an Electronic Health Record, and problem-solving within a clinical context.
• `Bonus`: A Bachelor’s degree in Nursing is preferred to further enhance the clinical and leadership capabilities in the role.
