Job Description:
This is a student intern position at Fresenius Medical Center, designed to support the FMCNA mission, vision, core values, and customer service philosophy, while adhering to compliance programs and policies. The intern will function as part of the hemodialysis healthcare team, providing safe and effective dialysis therapy to patients under the direct supervision of a licensed social worker.
Key Responsibilities Include (Under Supervision):
• Customer Service: Drive FMS culture through values and customer service standards, ensuring outstanding service to both external and internal customers.
• Patient Assessment/Care Planning/Counseling:
• Assessing patients’ psychosocial status as part of a comprehensive assessment.
• Participating in care planning with the healthcare team and patients to identify interventions for rehabilitation, treatment goals, and improved quality of life.
• Utilizing FMS Ultra Care programs and established social work methods.
• Providing established psycho-educational counseling to improve areas of need, treatment adherence, vocational/educational rehabilitation, and quality of life.
• Providing supportive counseling to transplant-seeking patients.
• Assessing patient awareness of advance directives and facilitating related discussions.
• Providing information and assistance with referrals to community resources.
• Addressing non-adherence and quality of life concerns.
• Adhering to FMCNA Social Work Policy, including documentation.
• Patient Education:
• Assessing patient knowledge of kidney disease and providing tailored education.
• Providing information about treatment modalities and facilitating the transplant referral process.
• Providing ongoing education about psychosocial issues related to ESRD and available support services.
• Reviewing patient rights and responsibilities and facility policies.
• Collaborating on QAI activities.
• Insurance and Financial Assistance:
• Maintaining knowledge of vocational/educational rehabilitation programs and assisting with referrals.
• Understanding FMCNA financial policies and educating patients about available programs and billing concerns.
• Staff Related:
• Maintaining updated patient resource lists.
• Developing and providing training to staff on psychosocial topics and Ultra Care Programs.
• Performing other duties as assigned.
Physical Demands and Working Conditions:
• Desk and computer work, interaction with patients, staff, and physicians.
• Travel between facilities and community locations may be required.
• Work environment is a healthcare facility with temperature control and moderate noise levels.
• Potential exposure to infectious and contagious diseases/materials.
Education:
• High School diploma or G.E.D.
Experience and Required Skills:
• Preferred outstanding customer service skills.
• Desire to deliver superior patient care and embrace Fresenius’ mission and values.
Relationships:
• The Intern cannot work under the supervision of a family member.
• The Intern cannot care for a patient who is a family member.
• If an Intern’s family member is a patient at the facility in which they work, the Intern will be scheduled to work on days the patient is not in the facility or will be assigned to another patient.
Supervision:
• The Clinical Manager assigns and closely monitors all SW Intern assignments with direct supervision from a SWFI based on the agreed education plan.
Other:
• Performs additional duties as assigned.
