Job Description: RN Case Manager, Home Health

The RN Case Manager in Home Health at CenterWell Home Health plays a crucial role in providing personalized, one-on-one nursing care to patients in their homes, aiming to help them regain independence. This position involves close collaboration with a dedicated team of physicians and clinicians to develop and manage effective care plans that support patient recovery.

Key Responsibilities Include:

Patient Admissions & Visits: Performing admission, ongoing case management, and follow-up skilled nursing visits for home health patients.
Care Plan Development & Management: Collaborating with physicians to develop the initial plan of treatment based on physician’s orders and initial patient assessment. Providing hands-on care, managing, and evaluating the care plan, and educating the patient. Revising the plan in consultation with the physician based on ongoing assessments and policy/regulation requirements, all under the supervision of a Clinical Manager.
Care Coordination: Coordinating appropriate care that involves various healthcare personnel, such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers, to ensure comprehensive patient support.
Reporting & Communication: Continuously reporting patient care, condition, and progress to both the patient’s physician and the Clinical Manager.
Patient & Family Engagement: Implementing the patient care plan in conjunction with the patient and their family to help them achieve optimal resolution of their needs and problems.
Supervision of Aides: Coordinating, overseeing, and supervising the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers. This includes providing written personal care instructions/care plans that reflect the current plan of care and monitoring the appropriate completion of documentation by these workers as part of the supervisory responsibility.
Patient Discharge: Discharging patients after consultation with the physician and Clinical Manager, ensuring all necessary clinical documentation is prepared and completed.
Documentation: Preparing comprehensive medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. This also involves preparing visit/shift reports, updating/summarizing patient records, and conferring with other healthcare disciplines to ensure optimum patient care.

Required Experience/Skills:

Education: Diploma, Associate, or Bachelor Degree in Nursing.
Experience: A minimum of one year of nursing experience is preferred, with strong experience in Med-Surg, ICU, ER, or acute care settings highly valued. Home Health experience is a plus.
Licensure & Certification: Current and unrestricted Registered Nurse licensure and current CPR certification.
Core Competencies: Strong organizational and communication skills.
Mobility: A valid driver’s license, auto insurance, and reliable transportation are required.

Pay & Benefits Information:

Scheduled Hours: Full-time, 40 hours per week.
Pay Structure: Compensation can be structured as pay per visit/unit, ranging from $45.00 – $63.00, or as an annual base pay.
Annual Base Pay Range: The estimated starting base pay for full-time employment ranges from $70,500 – $96,900 per year. This range is a good faith estimate at the time of posting and may vary based on geographic location, demonstrated job-related skills, knowledge, experience, education, and certifications.
