Job Description: Registered Nurse, Home Health

The Registered Nurse (RN) in Home Health at CenterWell Home Health provides comprehensive, personalized nursing care and case management directly to patients in their homes. This role is crucial in helping patients recover from various conditions and regain independence.

Summary of the Role:
The Home Health RN is responsible for developing and managing individualized care plans in close collaboration with physicians. They coordinate services with a range of other healthcare professionals, supervise home health aides, maintain thorough documentation, and continuously evaluate patient progress to ensure optimal recovery and independence. The position emphasizes making a meaningful impact daily by delivering one-on-one care that empowers patients to return to their preferred quality of life.

Key Responsibilities:

Patient Admissions & Case Management: Conduct admission assessments, provide ongoing case management, and perform follow-up skilled nursing visits for home health patients.
Care Plan Development & Management:
• Confer with physicians to develop the initial plan of treatment based on physician’s orders and initial patient assessment.
• Provide hands-on care, actively manage, and evaluate the care plan.
• Educate the patient and family in accordance with physician orders, under the supervision of the Clinical Manager.
• Revise the care plan in consultation with the physician based on ongoing assessments and as required by policy/regulation.
Interdisciplinary Coordination: Coordinate appropriate care services involving various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers, to ensure integrated patient care.
Reporting & Communication: Continuously report patient care, condition, and progress to the patient’s physician and Clinical Manager.
Patient & Family Collaboration: Implement the patient care plan in conjunction with the patient and their family to assist them in achieving optimal resolution of their needs and problems.
Supervision of Aides: Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers. This includes providing clear, written personal care instructions/care plans that reflect the current plan of care and monitoring the appropriate completion of documentation by these workers as part of the supervisory responsibility.
Patient Discharge: Discharge patients after consultation with the physician and Clinical Manager, ensuring all necessary clinical documentation is prepared and completed.
Documentation & Reporting: Prepare comprehensive medical documentation for all patients, which includes details on case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. This also involves preparing visit/shift reports, updating/summarizing patient records, and conferring with other healthcare disciplines to provide optimum patient care.

Required Experience/Skills:

Education: A Diploma, Associate, or Bachelor Degree in Nursing is required.
Nursing Experience: Minimum of one year of nursing experience is preferred. Strong experience in Med Surg, ICU, ER, or other acute care settings is highly valued. Home Health experience is a significant plus.
Licensure & Certifications: Must possess a current and unrestricted Registered Nurse (RN) licensure and current CPR certification.
Professional Skills: Strong organizational and communication skills are essential for this role.
Transportation: A valid driver’s license, auto insurance, and reliable transportation are mandatory for home visits.

Pay Range:

Per Visit/Unit Pay: $45.00 – $63.00.
Annual Base Pay: $70,500 – $96,900 per year for full-time (40 hours per week) employment.
