Job Description: Registered Nurse Home Health

The Registered Nurse Home Health at CenterWell Home Health delivers personalized nursing care directly to patients in their homes. This role is central to coordinating with various healthcare professionals to develop and manage individualized patient care plans, aiming for optimal patient recovery and independence.

Key Responsibilities and Duties:

Patient Admissions & Case Management: Provide skilled nursing visits for home health patients, including initial admissions, ongoing care, and case management.
Care Plan Development & Management:
• Confer with physicians to develop the initial plan of treatment based on physician’s orders and initial patient assessment.
• Provide hands-on care, manage and evaluate the care plan, and educate the patient in accordance with physician orders, under the supervision of a Clinical Manager.
• Revise care plans in consultation with the physician based on ongoing assessments and as required by policy/regulation.
Care Coordination: Coordinate appropriate care encompassing various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers to ensure comprehensive support.
Communication & Reporting: Continuously report patient care, condition, and progress to the patient’s physician and Clinical Manager.
Patient & Family Education: Implement the patient care plan in conjunction with the patient and family to assist them in achieving optimal resolution of their needs/problems.
Supervision of Home Health Aides: Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers. This includes providing written personal care instructions/care plans that reflect the current plan of care and monitoring their documentation for appropriate completion as part of supervisory responsibility.
Discharge Planning: Discharge patients after consultation with the physician and Clinical Manager, ensuring all necessary clinical documentation is prepared and completed.
Clinical Documentation: Prepare accurate and appropriate medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. This also involves preparing visit/shift reports, updating/summarizing patient records, and conferring with other healthcare disciplines to provide optimal patient care.

Required Experience and Skills:

To make an impact in this role, candidates should possess:

Education: A Diploma, Associate, or Bachelor Degree in Nursing.
Nursing Experience: Minimum of one year of nursing experience preferred.
Acute Care Experience: Strong experience in Med Surg, ICU, ER, or other acute care settings is highly valued.
Home Health Experience: Prior home health experience is a plus.
Licensure: Current and unrestricted Registered Nurse licensure.
Certifications: Current CPR certification.
Key Skills: Strong organizational and communication skills.
Transportation: A valid driver’s license, auto insurance, and reliable transportation are essential due to the nature of home visits.

Compensation:

The compensation for this role can be structured in two ways:

Per Visit/Unit Pay: $45.00 – $63.00 per visit/unit.
Annual Base Pay: $70,500 – $96,900 per year (for full-time, 40 hours per week employment). Individual pay may vary based on geographic location and demonstrated job-related skills, knowledge, experience, education, and certifications.
