Job Description: Registered Nurse, Home Health

Summary
The Registered Nurse, Home Health at CenterWell provides personalized nursing care and case management to patients in the comfort of their homes. This role is crucial in helping patients recover and regain independence. Key responsibilities include developing and managing care plans in close collaboration with physicians, coordinating services with other healthcare professionals, supervising home health aides, maintaining meticulous documentation, and continuously evaluating patient progress. The nurse makes a meaningful impact daily by providing one-on-one care that supports recovery and helps patients return to their desired lives.

Key Responsibilities

As a Home Health Registered Nurse, you will be responsible for:

Patient Admissions and Visits: Providing admission, ongoing case management, and follow-up skilled nursing visits for home health patients.
Care Plan Development & Management:
• Administering ongoing care and case management for each patient, including necessary follow-up as directed by the Clinical Manager.
• Collaborating with the physician to develop the initial plan of treatment based on physician’s orders and initial patient assessment.
• Delivering hands-on care, managing and evaluating the care plan, and providing patient education in accordance with physician orders, under the Clinical Manager’s supervision.
• Revising the care plan in consultation with the physician based on ongoing assessments and as required by policy/regulation.
Healthcare Coordination: Coordinating appropriate care that encompasses various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
Reporting & Communication: Continuously reporting patient care, condition, and progress to the patient’s physician and Clinical Manager.
Patient & Family Engagement: Implementing the patient care plan in conjunction with the patient and family to assist them in achieving optimal resolution of needs/problems.
Supervision & Oversight:
• Coordinating, overseeing, and supervising the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
• Providing written personal care instructions/care plan that reflect the current plan of care.
• Monitoring the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Patient Discharge: Discharging patients after consultation with the physician and Clinical Manager, preparing and completing all necessary clinical documentation.
Documentation & Collaboration:
• Preparing appropriate medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Preparing visit/shift reports, updating/summarizing patient records.
• Conferring with other healthcare disciplines to ensure optimum patient care.

Required Experience/Skills

To make an impact in this role, candidates should possess:

Education: A Diploma, Associate or Bachelor Degree in Nursing.
Nursing Experience: Minimum of one year nursing experience preferred.
Clinical Acuity: Strong med surg, ICU, ER, acute experience is highly valued.
Specialized Experience (Plus): Home Health experience is a plus.
Licensure & Certifications:
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
Professional Skills: Strong organizational and communication skills.
Mobility: A valid driver’s license, auto insurance, and reliable transportation are essential for home visits.

Compensation & Benefits

Pay Range: Compensation is offered in two structures:
Per Visit/Unit: $45.00 – $63.00.
Annual Base Pay: $70,500 – $96,900 per year for full-time (40 hours per week) employment. This range is an estimated starting base pay and may vary based on geographic location, demonstrated job-related skills, knowledge, experience, education, and certifications.
Scheduled Weekly Hours: 40 hours.
