Job Description: Registered Nurse, Home Health
The Registered Nurse (RN) in Home Health at CenterWell Home Health is a vital member of the care team, providing personalized, skilled nursing care, and comprehensive case management directly in patients’ homes. The primary goal of this role is to facilitate patient recovery and promote independence within the comfort of their own environment. RNs collaborate closely with physicians and multidisciplinary teams to develop, implement, and continuously revise individualized care plans to ensure optimal health outcomes for each patient. A key supervisory aspect of the role involves overseeing home health aides.
Key Responsibilities of a Home Health Registered Nurse include:
• Patient Admissions and Case Management: Conducting admission visits, providing ongoing case management, and performing follow-up skilled nursing visits for home health patients, with direction from the Clinical Manager.
• Care Plan Development and Implementation: Conferring with physicians to develop initial plans of treatment based on physician orders and initial patient assessments. Providing hands-on care, managing and evaluating the care plan, and educating the patient in accordance with physician orders and under the Clinical Manager’s supervision. Revising the care plan in consultation with the physician based on ongoing assessments and regulatory requirements.
• Interdisciplinary Coordination: Coordinating appropriate care that involves various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external healthcare providers, ensuring a cohesive and comprehensive approach to patient care.
• Reporting and Communication: Continuously reporting patient care details, condition, and progress to the patient’s physician and the Clinical Manager to ensure timely and informed decision-making.
• Patient and Family Engagement: Implementing the patient care plan in conjunction with the patient and their family, empowering them to actively participate in achieving optimal resolution of their health needs and problems.
• Supervision of Support Staff: Coordinating, overseeing, and supervising the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers. This includes providing written personal care instructions/care plans that reflect the current plan of care and monitoring the appropriate completion of documentation by these staff members as part of the supervisory and leadership responsibilities.
• Patient Discharge: Discharging patients after consultation with both the physician and Clinical Manager, and ensuring all necessary clinical documentation is accurately prepared and completed.
• Comprehensive Documentation: Preparing appropriate medical documentation for all patients, which includes details of any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan modifications. This also involves preparing visit/shift reports, updating/summarizing patient records, and conferring with other healthcare disciplines to provide optimum patient care.
Required Qualifications and Skills:
• A Diploma, Associate, or Bachelor Degree in Nursing.
• A minimum of one year of nursing experience is preferred, with strong experience in Med-Surg, ICU, or ER (acute care settings) being highly valued.
• Home Health experience is considered a significant plus.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Strong organizational and communication skills.
• A valid driver’s license, auto insurance, and reliable transportation are essential for home visits.
Compensation Structure:
The role offers competitive compensation with two options:
• Per Visit/Unit Pay: $45.00 – $63.00 per visit/unit.
• Annual Base Pay: $70,500 – $96,900 per year for full-time employment (40 scheduled weekly hours).
The compensation range is an estimated starting base pay and may vary based on geographic location and individual qualifications such as demonstrated job-related skills, knowledge, experience, education, and certifications.
