Job Description: Registered Nurse Home Health at CenterWell Home Health
Job Summary
A Registered Nurse Home Health at CenterWell Home Health provides personalized nursing care and case management to patients within the comfort of their own homes. The role’s primary focus is to facilitate patient recovery and promote independence. This nurse collaborates closely with physicians and other healthcare professionals to develop, implement, and revise comprehensive care plans. They also supervise home health aides and ensure meticulous clinical documentation. This position demands strong clinical skills, excellent communication abilities, current nursing licensure, and experience in both acute care settings and ideally, home health. The role offers the opportunity to make a meaningful daily impact by providing one-on-one care that helps patients regain independence and return to their desired lives.
Key Responsibilities
As a Home Health Registered Nurse, you will:
• Provide admission, ongoing case management, and follow-up skilled nursing visits for home health patients.
• Administer continuous care and case management for each patient, including necessary follow-up as directed by the Clinical Manager.
• Confer with the physician to develop the initial plan of treatment based on physician’s orders and the initial patient assessment.
• Provide hands-on care, manage and evaluate the care plan, and educate the patient in accordance with physician orders, under the Clinical Manager’s supervision.
• Revise the care plan in consultation with the physician based on ongoing assessments and as required by policy/regulation.
• Coordinate appropriate care involving various healthcare personnel, such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Continuously report patient care, condition, and progress to the patient’s physician and Clinical Manager.
• Implement the patient care plan in conjunction with the patient and family to help them achieve optimal resolution of needs/problems.
• Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers. This includes providing written personal care instructions/care plans that reflect the current plan of care and monitoring the appropriate completion of documentation by these workers as part of the supervisory responsibility.
• Discharge patients after consultation with the physician and Clinical Manager, ensuring all needed clinical documentation is prepared and completed.
• Prepare appropriate medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimum patient care.
Required Experience/Skills
• Education: Diploma, Associate, or Bachelor Degree in Nursing.
• Nursing Experience: Minimum of one year of nursing experience preferred.
• Acute Care Experience: Strong experience in Med-Surg, ICU, ER, or other acute care settings is required.
• Home Health Experience: Home Health experience is considered a plus.
• Licensure: Current and unrestricted Registered Nurse licensure.
• Certifications: Current CPR certification.
• Skills: Strong organizational and communication skills.
• Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Compensation
The compensation structure offers two options:
• Per Visit/Unit Pay: $45.00 – $63.00 per visit/unit.
• Annual Base Pay: $70,500 – $96,900 per year.
• This range reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The actual pay may vary based on geographic location and individual factors such as demonstrated job-related skills, knowledge, experience, education, and certifications.
• Scheduled Weekly Hours: 1 (Note: This might indicate flexibility or per diem nature, but the annual base pay suggests full-time potential as well, based on the description of the pay range).
