Job Description: Registered Nurse Home Health
This role as a Registered Nurse in Home Health at CenterWell Home Health involves providing personalized nursing care to patients directly in their homes. The primary goal is to help patients recover, regain independence, and improve their quality of life. This position requires collaboration with physicians and a multidisciplinary healthcare team to develop, manage, and evaluate comprehensive care plans.
Key Responsibilities:
• Conduct admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
• Confer with physicians to develop initial plans of treatment based on physician’s orders and initial patient assessments.
• Provide hands-on care, management, and evaluation of the care plan, including patient teaching, in accordance with physician orders and under Clinical Manager’s supervision.
• Revise care plans in consultation with physicians based on ongoing assessments and policy/regulation requirements.
• Coordinate appropriate care encompassing various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Report patient care, condition, and progress to the patient’s physician and Clinical Manager on a continuous basis.
• Implement patient care plans in conjunction with the patient and family to help them achieve optimal resolution of needs/problems.
• Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers. This includes providing written personal care instructions/care plans that reflect the current plan of care and monitoring their documentation completion.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing all necessary clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. This also involves preparing visit/shift reports, updating/summarizing patient records, and conferring with other healthcare disciplines for optimum patient care.
Required Experience/Skills:
• Education: Diploma, Associate, or Bachelor Degree in Nursing.
• Nursing Experience: Minimum of one year of nursing experience preferred.
• Specialized Experience: Strong experience in Med Surg, ICU, ER, or acute care is highly valued. Home Health experience is a plus.
• Licensure: Current and unrestricted Registered Nurse licensure.
• Certifications: Current CPR certification.
• Professional Skills: Strong organizational and communication skills.
• Transportation: Valid driver’s license, auto insurance, and reliable transportation are essential.
Compensation:
• Scheduled Weekly Hours: 1 (Note: This might indicate a PRN/per diem role or is a typo for full-time. The pay range below suggests a full-time equivalent).
• Pay Range: The estimated starting base pay for full-time (40 hours per week) employment ranges from $70,500 to $96,900 per year. Actual pay may vary based on geographic location, demonstrated job-related skills, knowledge, experience, education, and certifications.
