Job Description: Registered Nurse Home Health
The Registered Nurse Home Health role at CenterWell Home Health is centered on providing personalized, one-on-one nursing care to patients in their homes. The primary goal is to help patients regain independence and recover comfortably, working closely with a dedicated team of physicians and clinicians to develop and manage individualized care plans. This role makes a meaningful impact by focusing on optimal patient recovery and independence.
Key Responsibilities of a Home Health Registered Nurse:
• Patient Care and Case Management:
• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer on-going care and case management for each patient, with necessary follow-up as directed by the Clinical Manager.
• Confer with the physician to develop the initial plan of treatment based on physician’s orders and initial patient assessment.
• Provide hands-on care, management, and evaluation of the care plan, including teaching the patient in accordance with physician orders, under the Clinical Manager’s supervision.
• Revise the plan of care in consultation with the physician based on ongoing assessments and policy/regulation requirements.
• Implement the patient care plan in conjunction with the patient and family to assist them in achieving optimal resolution of needs/problems.
• Coordination and Communication:
• Coordinate appropriate care, encompassing various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Report patient care/condition/progress to the patient’s physician and Clinical Manager on a continuous basis.
• Supervision and Documentation:
• Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions/care plans that reflect the current plan of care.
• Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other health care disciplines to provide optimum patient care.
Required Experience/Skills:
• Education: Diploma, Associate, or Bachelor Degree in Nursing.
• Nursing Experience: Minimum of one year of nursing experience is preferred. Strong experience in med surg, ICU, ER, or acute care settings is highly valued. Home Health experience is a plus.
• Licensure & Certifications:
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Key Competencies: Strong organizational and communication skills.
• Logistics: Valid driver’s license, auto insurance, and reliable transportation are essential.
Pay Range:
The compensation for this role offers flexibility and is provided in two formats:
• Per visit/unit: $45.00 – $63.00.
• Per year base pay: $70,500 – $96,900. This range reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. Individual pay may vary based on geographic location, demonstrated job-related skills, knowledge, experience, education, and certifications.
