This is a part-time opportunity for a Nurse Practitioner (NP) or Physician Assistant (PA) to work as an advanced practice clinician within post-acute care communities.
Key Responsibilities:
• Drive high-quality, safe, and cost-effective patient care.
• Perform inpatient rounding in skilled nursing facilities, inpatient rehabilitation facilities, and long-term care hospitals.
• Collaborate with other clinicians and interdisciplinary teams, adhering to CMS guidelines.
• Work closely with hospice, palliative care, and home care agencies to ensure safe patient transitions.
• Maintain timely documentation and billing through the EMR (Gehrimed), meeting Medicare guidelines.
Patient Volume:
• Expected to see 12 patients per day.
Compensation and Benefits:
• Estimated annual income ranges from $63,000 to $75,000, with no cap on productivity bonus income potential.

Qualifications:
• Board certification as an FNP, AGACNP, ACNP, or AGPCNP.
