Job Description: Licensed Practical Nurse Home Health

As a Licensed Practical Nurse (LPN) in Home Health at CenterWell Home Health, you will provide personalized nursing care to patients in their homes. The primary goal of this role is to support patient recovery and independence under the direct supervision of Registered Nurses (RNs) and physicians. This position involves delivering skilled nursing interventions, assisting in care coordination, meticulously maintaining documentation, and ensuring strict adherence to professional and regulatory standards. It requires a blend of clinical experience, current state licensure, and a collaborative spirit within a multidisciplinary healthcare team to promote optimal patient well-being and recovery.

Key Responsibilities and Activities of a Home Health LPN include:

Professional Nursing Services: Providing professional nursing services under the direction of an RN, ensuring full compliance with the state’s Nurse Practice Act, all applicable licensure/certification requirements, healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
Skilled Nursing Interventions: Delivering skilled nursing interventions for the treatment of patient illnesses, addressing rehabilitative needs, and performing preventative care. This involves utilizing a holistic approach in problem-specific therapeutic interventions, teaching, and training activities, all in strict accordance with the established plan of care.
Clinical Practice Application: Applying knowledge and skills in line with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
Care Plan Collaboration: Conferring with the RN Supervisor regarding any necessary changes in the Plan of Treatment. This role also involves accepting verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act and promptly communicating these orders to the RN Supervisor.
Nursing Plan Implementation: Utilizing a systematic, individualized, goal/outcome-driven approach in implementing the nursing plan of care.
Communication and Coordination: Maintaining timely contact with patients, physicians, clinical manager(s), and other members of the healthcare team regarding patient/family needs and any status changes. The LPN will also participate in care coordination activities and discharge planning as appropriate.
Confidentiality: Upholding the highest standards of professional conduct concerning confidential information, sharing information only when the recipient’s right to access is clearly established and when sharing such information is unequivocally in the best interests of the patient.
Resource Management: Communicating appropriately to ensure adherence to professional standards in the provision and availability of supplies, materials, and equipment necessary to safely and effectively implement the plan of care.
Documentation: Preparing, submitting, and maintaining all required documentation as mandated by the Company and/or facility, ensuring that visit/shift notes are documented on the same day services are rendered.
