Job Description: Licensed Practical Nurse, Home Health
This role as a Licensed Practical Nurse (LPN) in Home Health at CenterWell Home Health involves providing personalized, one-on-one nursing care and support to patients within the comfort of their homes. LPNs work under the direct supervision of registered nurses and physicians, playing a crucial role in helping patients regain independence and improve their overall well-being.
Summary of the Role:
The Home Health LPN develops and manages individualized care plans to promote recovery, independence, and overall well-being. Responsibilities encompass a range of skilled nursing interventions, patient education, care coordination, and thorough documentation, all in strict compliance with healthcare standards and regulations. The role offers a meaningful opportunity to make a daily impact as part of a dedicated team of physicians and clinicians.
Key Responsibilities and Duties:
• Professional Nursing Services: Provide nursing care under the direction of a Registered Nurse (RN), adhering to the state’s Nurse Practice Act, applicable licensure/certification requirements, healthcare standards, governmental laws, regulations, and CenterWell Home Health policies and procedures.
• Skilled Nursing Interventions: Deliver skilled nursing interventions for patient illness treatment, rehabilitative needs, and preventative care. This involves utilizing a holistic approach, applying problem-specific therapeutic interventions, and conducting teaching and training activities in accordance with the established plan of care.
• Clinical Practice Application: Apply knowledge and skills in line with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Care Plan Collaboration: Confer with the RN Supervisor regarding necessary changes to the Plan of Treatment. Accept verbal orders from physicians where permitted by state law/regulations/Nurse Practice Act, and promptly communicate these orders to the RN Supervisor.
• Care Plan Implementation: Employ a systematic, individualized, goal/outcome-driven approach when implementing the nursing plan of care.
• Communication and Coordination: Maintain timely contact with patients, physicians, clinical manager(s), and other healthcare team members regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
• Confidentiality: Uphold the highest standards of professional conduct concerning confidential information, sharing it only when the recipient’s right to access is clearly established and it is in the patient’s best interest.
• Resource Management: Appropriately communicate to ensure adherence to professional standards regarding the provision and availability of supplies, materials, and equipment necessary for safe and effective implementation of the plan of care.
• Documentation: Prepare, submit, and maintain all required documentation as mandated by the Company and/or facility. Visit/shift notes must be accurately documented on the day services are rendered.
Required Experience/Skills:
• Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Licensure: Current nursing license in the practicing state.
• Transportation: Valid driver’s license, active auto insurance, and reliable transportation.
• Certifications: Current CPR certification.
• Experience: A minimum of two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice environment.
Work Schedule:
• The position is full-time, with a scheduled commitment of 40 Weekly Hours.
Pay Range:
• The estimated starting base pay for full-time employment is between $56,200 and $77,400 per year.
