Job Description: Licensed Practical Nurse, Home Health

The Licensed Practical Nurse (LPN) in Home Health provides personalized nursing care and support directly to patients in their homes. This role operates under the supervision of registered nurses and physicians. A key aspect of the role involves developing and managing individualized care plans aimed at promoting patient recovery, independence, and overall well-being. Responsibilities encompass skilled nursing interventions, comprehensive patient education, care coordination, and meticulous documentation, all in strict compliance with healthcare standards and regulations. As a Home Health LPN, You Will:

• Provide professional nursing services under the direct supervision of a Registered Nurse (RN), ensuring compliance with the state’s Nurse Practice Act, applicable licensure/certification requirements, healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
• Execute skilled nursing interventions tailored to the patient/client’s illness, rehabilitative needs, and preventative care.
• Utilize a holistic approach to deliver problem-specific therapeutic interventions, teaching, and training activities in accordance with the established plan of care.
• Apply knowledge and skills in line with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
Confer with the RN Supervisor regarding necessary changes in the Plan of Treatment. Accept verbal orders from physicians where permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.
• Implement the nursing plan of care using a systematic, individualized, goal/outcome-driven approach.
• Maintain timely contact with patients, physicians, clinical manager(s), and other members of the healthcare team concerning patient/family needs and changes in status.
• Participate in care coordination activities and discharge planning as appropriate.
• Uphold the highest standards of professional conduct concerning confidential patient information, sharing it only when the recipient’s right to access is clearly established and doing so is demonstrably in the patient’s best interest.
• Communicate appropriately to ensure adherence to professional standards regarding the provision and availability of supplies, materials, and equipment necessary to safely and effectively implement the plan of care.
• Prepare, submit, and maintain all required documentation as mandated by the Company and/or facility, ensuring visit/shift notes are documented on the day services are rendered.

Required Experience/Skills:

Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Possession of a current nursing license in the practicing state.
Transportation: A valid driver’s license, auto insurance, and reliable transportation are essential.
Certification: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice setting.

Scheduled Weekly Hours: 40 hours

Pay Range: The estimated starting base pay for full-time (40 hours per week) employment ranges from $56,200 to $77,400 per year. Actual pay may vary based on geographic location, demonstrated job-related skills, knowledge, experience, education, and certifications.
