Job Description: Licensed Practical Nurse, Home Health

This role involves providing personalized, one-on-one nursing care to patients in their homes under the supervision of registered nurses and physicians. The primary goal is to help patients regain independence, facilitate recovery, and promote preventive care. LPNs work closely with a dedicated team of physicians and clinicians to develop and manage individualized care plans.

Key Responsibilities:

Professional Nursing Services: Provide skilled nursing interventions in compliance with state Nurse Practice Acts, licensure requirements, healthcare standards, governmental laws, regulations, and CenterWell Home Health policies.
Patient Care & Interventions: Deliver skilled nursing interventions for illness treatment, rehabilitative needs, and preventive care. Utilize a holistic approach, applying problem-specific therapeutic interventions, teaching, and training activities in accordance with the established plan of care.
Clinical Application: Apply clinical knowledge and skills according to accepted standards of practice to resolve patient problems and achieve individualized goals and outcomes.
Collaboration & Communication: Confer with the RN Supervisor regarding necessary changes to the Plan of Treatment. Accept verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act and communicate these to the RN Supervisor. Maintain timely contact with patients, physicians, clinical managers, and other healthcare team members regarding patient/family needs and status changes. Participate in care coordination and discharge planning as appropriate.
Documentation: Prepare, submit, and maintain accurate and detailed documentation as required by the company and/or facility, ensuring visit/shift notes are documented on the day services are rendered.
Professional Conduct: Uphold the highest standards of professional conduct, especially regarding confidential patient information, sharing it only when access is clearly established and beneficial to the patient.
Resource Management: Ensure appropriate communication regarding the availability of supplies, materials, and equipment needed for safe and effective plan of care implementation.

Required Experience/Skills:

Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Current nursing license in the practicing state.
Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Certification: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice setting.

Compensation and Benefits:

Pay Range: The role offers a base pay range of $44,600 – $61,400 per year for full-time (40 hours per week) employment. Additionally, there is a pay-per-visit/unit option ranging from $28.00 – $40.00. Actual pay may vary based on geographic location, job-related skills, knowledge, experience, education, and certifications.
Scheduled Weekly Hours: 40 hours.
