Job Description: Licensed Practical Nurse Home Health
The Licensed Practical Nurse (LPN) in Home Health at CenterWell Home Health provides personalized nursing care to patients directly in their homes. This role is crucial in supporting patient recovery and fostering independence, always under the supervision of Registered Nurses (RNs) and physicians. The LPN delivers skilled nursing interventions, assists with care coordination, maintains thorough documentation, and ensures strict adherence to professional and regulatory standards.
Key Responsibilities and Duties:
• Professional Nursing Services: Provide nursing care under the direct supervision of a Registered Nurse (RN), ensuring full compliance with the state’s Nurse Practice Act, all applicable licensure and certification requirements, healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
• Skilled Nursing Interventions: Deliver skilled nursing interventions aimed at treating patient/client illnesses, addressing rehabilitative needs, and providing preventative care. Utilize a holistic approach by applying problem-specific therapeutic interventions, teaching, and training activities in accordance with the established plan of care.
• Clinical Practice Application: Apply clinical knowledge and skills in line with accepted standards of clinical practice to facilitate problem resolution and help patients achieve their individualized goals and desired outcomes.
• Plan of Treatment Collaboration: Confer with the RN Supervisor regarding any necessary changes to the Plan of Treatment. Accept verbal orders from physicians where permitted by state law, regulations, and the Nurse Practice Act, and promptly communicate these orders to the RN Supervisor.
• Care Plan Implementation: Utilize a systematic, individualized, and goal/outcome-driven approach in the implementation of the nursing plan of care.
• Communication and Coordination: Maintain timely contact with patients, physicians, clinical manager(s), and other members of the healthcare team regarding patient/family needs and any changes in status. Participate in care coordination activities and discharge planning as appropriate.
• Confidentiality: Uphold the highest standards of professional conduct concerning confidential patient information. Share information only when the recipient’s right to access is clearly established and when sharing such information is unequivocally in the patient’s best interest.
• Resource Management: Communicate appropriately to ensure adherence to professional standards regarding the provision and availability of supplies, materials, and equipment necessary to safely and effectively implement the plan of care.
• Documentation: Prepare, submit, and maintain all required documentation as mandated by the Company and/or facility. Visit/shift notes must be documented on the same day services are rendered.
Required Experience/Skills:
• Education: Graduation from an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Licensure: Current nursing license in the practicing state.
• Transportation: Valid driver’s license, auto insurance, and reliable transportation.
• Certifications: Current CPR certification.
• Experience: Two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice setting.
Compensation:
• Pay Structure: This role offers a pay range of $28.00 – $40.00 per visit/unit for services rendered.
• Base Pay (Full-Time): For full-time employment (40 hours per week), the estimated starting base pay ranges from $44,600 – $61,400 per year. The actual pay may vary based on geographic location, demonstrated job-related skills, knowledge, experience, education, and certifications.
• Scheduled Weekly Hours: 40 hours.
