About the Job: CenterWell Home Health Nurse

This role as a CenterWell Home Health Registered Nurse is an integral part of a caring community focused on patient health and independence. Nurses provide personalized, one-on-one care in patients’ homes, collaborating with physicians and other clinicians to develop and manage care plans that support recovery and improve quality of life.

Key Responsibilities of a Home Health Registered Nurse:
• Conduct admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management, including necessary follow-up, under the direction of the Clinical Manager.
• Collaborate with physicians to develop initial plans of treatment based on orders and patient assessment.
• Provide hands-on care, manage and evaluate care plans, and educate patients in accordance with physician orders, under Clinical Manager supervision. Revise plans in consultation with physicians as needed.
• Coordinate comprehensive care involving various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Continuously report patient care, condition, and progress to the patient’s physician and Clinical Manager.
• Implement patient care plans collaboratively with the patient and family to achieve optimal resolution of needs and problems.
• Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions and care plans.
• Monitor the appropriate completion of documentation by home health aides/personal care workers as part of supervisory responsibilities.
• Discharge patients after consultation with the physician and Clinical Manager, ensuring all necessary clinical documentation is prepared and completed.
• Prepare comprehensive medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, care plan changes, visit/shift reports, and updates/summaries of patient records. Confer with other healthcare disciplines for optimal patient care.

Required Experience/Skills:
Education: Diploma, Associate, or Bachelor Degree in Nursing.
Experience: Minimum of one year nursing experience is preferred. Strong experience in med-surg, ICU, ER, or acute care is highly valued. Home Health experience is a plus.
Licensure/Certifications: Current and unrestricted Registered Nurse licensure, along with current CPR certification.
Skills: Strong organizational and communication skills are essential.
Logistics: Valid driver’s license, auto insurance, and reliable transportation are required.
Work Schedule: The scheduled weekly hours for this position are 40.
